At Revive Clinic, we pride ourselves on providing the best services, care, and experience for all of our patients. The following policies help ensure that each visit is up to our high standard of making our patients our number one priority.
Appointment Policy
New patients must arrive on time in order to receive the full duration of their scheduled consultation. If new patient paperwork has not been completed ahead of time, we kindly ask that you arrive at least 15 minutes early so our staff can assist you.
For established patients, we allow a 10-minute grace period. If you arrive more than 10 minutes late, your appointment may need to be rescheduled, and a $50 rescheduling fee will be applied.
Cancellation & No-Show Policy
To respect the time of our providers and staff, we ask that any cancellations or rescheduling be made at least 24 hours in advance. Any appointment that is cancelled with less than 24 hours’ notice or results in a no-show will incur a $50 fee, which will be automatically charged to the card on file.
Extended Appointment Policy (2+ Hours)
For appointments lasting 2 hours or more, we require a minimum of 48 hours’ notice for cancellations or changes. Cancellations under 48 hours or no-shows for these extended appointments will result in a $150 fee, automatically charged to the card on file. In addition, some longer or high-value services may require full prepayment at the time of booking.
Refund Policy
All payments for any product or service are considered non-refundable. If you make a full payment or deposit and later decide not to proceed with the original product or service, the payment may be applied toward another product or service with the same provider you were originally scheduled with.
If you pre-pay for a service, a series of services, or have a credit on your account, the credit or service must be used within 12 months of the purchase date.
*All patients will be required to have a credit card on file. This card will be charged in the event of a no-show. You will not be able to reschedule your appointment until the $50/$150 no-show fee is paid.*
